NEWRAD 2011, Maui Hawaii

Please follow the link below to the computer fillable PDF Registration Form.  In addition to the items indicated in the Registration Form, the registration fee includes:

• refreshments during coffee breaks
• lunch on three days
  
• extended abstracts in USB
  

You may make reservations for the Conference and the Optional Conference Sponsored Events (Welcome Mixer, Luau, Haleakala Excursion, Sunset Cruise) at any time by using this form.   Optional Event registration is not required at this time. However, attendance is limited so make your reservations early.  Please use this same PDF form if you choose to register for the Optional Events at a later date.

Return the completed Registration Form (all pages) to us via email or fax.  We suggest using facsimile to return your form, as it is the most secure method.  Email offers little or no security for your credit card information.  The return email address and Fax number are located at the end of the PDF form.

All credit card charges will be processed as US dollars.
